Dr. John "Jack" R. Guy Ii

Dr. John “Jack” R. Guy II, a longtime Waukesha pediatrician, died peacefully surrounded by his family on Thursday, April 16, 2015 at Avalon Square at the age of 88. He was born in Boston, MA on December 25, 1926 the son of Emmett and Janet (nee Jones) Guy. Jack grew up in Whitefish Bay graduating from Whitefish Bay High School in 1944. He joined the Navy Air Corp and after an honorable discharge he graduated from the University of Wisconsin with his bachelor’s degree in 1948 and earned his M.D. in 1950. On June 26, 1948 he married Molly King in Wheaton, IL. Jack interned at Lutheran Hospital in Eau Claire and then for six years, he worked in general practice in Barron, WI. He went on to do a pediatric residency at Children’s Hospital in 1959 and moved to Waukesha where he worked as a pediatrician at Waukesha Pediatric Associates for many years. Jack served as chairman of many organizations including the Waukesha County Health Department, the Wisconsin Chapter of the American Academy of Pediatrics, Waukesha Memorial Hospital Department of Pediatrics and Waukesha Memorial Hospital Department of Neonatology. He was active in the American Medical Association, the Wisconsin State Medical Society, the Waukesha County Medical Society, American Academy of Pediatrics, the Milwaukee Pediatric Society, was past member of the Comprehensive Health Planning Agency of SEWI and was member of two Governor’s Commissions for Health Care Standards for Children in the State of Wisconsin. Jack also was an assistant clinical professor for the Medical College of Wisconsin, taught for the Department of Pediatrics for Children’s Hospital of Wisconsin and for the Family Residency Program at Waukesha Memorial Hospital. Jack was also active in AFS since 1965, having three foreign exchange students over the years and did medical consulting for the Waukesha AFS Chapter. He also enjoyed donating his time doing physical exams for athletes in the Waukesha Schools.

Jack was a longtime member of St. Matthias Episcopal Church. He was an avid skier, loved gardening and enjoyed hunting, fishing and sailing. Most of all he loved being with his family.

He will be sadly missed by his wife of 66 years, Molly of Waukesha and their children, Ted (Susann) Guy of Basalt, CO, Janet Berklund of Waukesha, Michael (Lynn) Guy of Edwards, CO and John R. (Mary) Guy, III of Whitefish Bay. He was the dear grandfather of Andrew (Shelly) Berklund of Waukesha, Shannon (Derek) Slicker of Waukesha, Alexandra (Kendall) Strand of Castle Rock, CO, John R. Guy IV of Tucson, AZ and Catherine Guy of Whitefish Bay and great-grandfather of Nate, Emma, Natalie, Joey, Hannah, Jocelyn, Claire and one more on the way. He is further survived by his sister-in-law Loretta Guy along with many cousins, nieces, nephews, other relatives and dear friends. He was preceded in death by his parents and his brother Tom Guy.

Visitation will be held on Saturday, April 25, 2015 from 12:30PM until the time of the 2:00PM memorial Mass at St. Matthias Episcopal Church, 111 E. Main Street, Waukesha, WI 53186. The Reverend David Simmons will preside. Inurnment will follow in the church columbarium. In lieu of flowers, memorials are appreciated to the Alzheimer’s Association Southeastern WI Chapter, 620 S. 76th St., Milwaukee, WI 53214 or to St. Matthias Episcopal Church (address above).
